 What Makes an RPO Firm "Healthcare Specialized"?

A hospital HR leader searching for "RPO firms that specialize in healthcare hiring" is usually not browsing out of curiosity. They are typically staring at a staffing report that shows what the rest of the industry is also seeing right now: RN turnover climbed back up to 17.6% nationally in 2025 after a few years of improvement, and the average hospital is still carrying 43 unfilled RN FTEs at any given time, according to NSI Nursing Solutions' 2026 retention report. Add in a national RN vacancy rate near 8.6% and an average cost of $60,090 per RN who leaves, and the math behind "we need a partner who actually knows healthcare" becomes obvious fast.

Healthcare Hiring Runs on Different Rules Than General Recruiting 

A generalist recruiter can source and screen a marketing manager or a warehouse associate using the same basic playbook across industries. Clinical hiring does not work that way. Licensure verification, credentialing timelines, specialty-specific sourcing, and compliance with accreditation standards (including The Joint Commission's newly formalized staffing performance goals) all add layers that a non-specialized recruiting process is not built to absorb.

That complexity shows up directly in the numbers. The average time to fill a medical/surgical RN role runs 80 to 109 days, and telemetry, PCU, and med/surg roles are consistently among the hardest to place. Even emergency department nurses, the fastest role to fill, still take more than two months on average.

These are the natural result of a talent pool that is licensed, geographically constrained, and in high demand across every health system at once. A firm without dedicated healthcare talent acquisition infrastructure is working the same slow math as an internal team stretched too thin.

The Real Cost of Getting It Wrong

For most CHROs and CFOs, the business case for a specialized partner comes down to premium labor. The average travel nurse costs roughly $91 per hour, compared to about $59 per hour for an employed staff RN, including benefits, a gap of more than $66,000 per nurse per year. Multiply that across a system carrying dozens of open positions, and premium labor spend becomes one of the largest controllable line items on the budget.

Three things tend to distinguish firms that genuinely specialize in healthcare hiring from RPO providers offering healthcare as one vertical among many.

Credentialing and licensure fluency. A specialized partner builds screening and sourcing workflows around license verification, specialty certifications, and state-specific requirements from day one, rather than bolting compliance onto a generic process after the fact.

Clinical role depth. Nursing, allied health, and provider recruiting each carry distinct candidate behavior, competitive dynamics, and urgency. A firm that has only recruited corporate or administrative roles will not have built the sourcing muscle for a med/surg RN search or a hard-to-fill APP role.

Data tied to healthcare-specific outcomes. Ask for proof points measured in the metrics that matter to a health system: vacancy rate, time-to-fill by role type, premium labor reduction, and 90-day retention, not generic recruiting statistics borrowed from other industries.

Questions to Ask Before Choosing a Healthcare RPO Partner 

  • Can you show fill rate and time-to-fill improvements broken out by clinical role type, not just an aggregate average?
  • How does your screening process specifically handle licensure and credentialing compliance?
  • What is your track record in reducing reliance on travel and agency labor, and over what timeframe?
  • Who makes the final hiring decision on every candidate, and how is that documented for audit purposes?
  • What does implementation actually look like for a health system your size, and how long until measurable impact?

The answers to those questions will tell a health system far more than any list of "top RPO firms"  search results.

Specialization in Hospital RPO and clinical talent acquisition is a function of process design, technology, and track record, not a category label a vendor assigns to itself.
